Ovarian germ cell tumor

The scoring system similar to that used in Cuiwan tumor can be used to evaluate the prognosis of ovarian germ cell tumor. Many studies have shown that FIGO staging, elevated levels of tumor markers, non-dysgerminoma or immature teratoma, sarcoma-like tissue and lymph node metastasis often have a poor prognosis.

Murugaesu et al reviewed 113 cases of ovarian germinoma and found that if the levels of AFP and β-hCG increased, the overall survival rate decreased significantly: if the tumor marker level was normal, the 1-year survival rate was 89.6%. If the tumor markers AFP and β-hCG were both positive, the 1-year survival rate decreased to 50.4%. Although the study included 113 cases, it was not sufficient to establish criteria for AFP and β-hCG. Lai et al reviewed 93 cases and suggested that the 5-year survival rate of dysgerminoma or immature teratoma was 100%, while the total survival rate of other types of tumors was 83.3%. This confirms that the prognosis of endodermal sinus tumor or choriocarcinoma is even worse. Malagon et al reviewed 46 cases of germinoma with sarcoma component (such as embryonic rhabdomyosarcoma, angiosarcoma and leiomyosarcoma) and compared with germ cell tumor cases of the same age and stage without this factor. It was found that tumors without sarcoma were more effective than tumors with sarcoma.

Recently, Kumar and colleagues found in a SEER study that regardless of age, stage, grade, and history, the mortality rate nearly tripled if there was lymph node metastasis (HR = 2.87, 95%CI: 1.44-5.73). In addition, the data also showed that the 5-year survival rate of patients who underwent lymphadenectomy was 94%, while that of patients without lymphadenectomy was 89%, suggesting that lymphadenectomy may improve the chances of survival. It is well known that tumor grade is related to lymph node metastasis, but there is no significant correlation. However, O'Connor et al found that tumor grade is the most important indicator of survival and prognosis of immature teratoma. Age is not an independent predictor of the prognosis of ovarian germinoma. A pediatric group studied 109 cases of extragonadal germinoma and found that the overall survival rate decreased significantly when the diagnosed age was ≥ 12 years old.
