Differential diagnosis of breast fibroadenoma

August 16, 2023

Breast fibroadenoma is the most common benign tumor of the breast. It is more common in young women under the age of 30, and it is rare after menopause. 15% of them are multiple. Female estrogen disorder is related to the disease. X-ray and USG are the main examination methods of the disease, and CT and MRI are helpful for differential diagnosis.

X-ray molybdenum target plain film showed a quasi-round mass in the breast, with smooth edges, uniform density, and often accompanied by halo. Sometimes massive calcification can be seen. The size of the tumor was consistent with the size of palpation.

CT examination showed that the morphological appearance was similar to that of X-ray plain film, and the CT value of enhanced scan was less than 40Hu.

MRI examination: the signal intensity of the tumor was related to the tissue composition. On plain T1WI, the tumor showed low signal or iso-signal circle, oval mass and clear edge. The fibroma with high fiber content on T2WI showed low signal intensity and moderate signal intensity, which is the characteristic manifestation of fibroadenoma. Fibromas with high cell and water content showed high signal intensity. The calcification of fibroma showed no signal on T1WI and T2WI. The ADC value of fibroadenoma was higher on DWI. Most of the dynamic MRI enhanced scans showed slow and gradual uniform enhancement or centrifugal enhancement with diffusion from the center to the outside, and a few showed rapid enhancement. In the differential diagnosis of breast fibroadenoma, it is necessary to combine the changes of morphological features and the changes of DWI and MRS to reduce misdiagnosis.
